Direct Billing Important Information
By using our direct billing services, you acknowledge and agree to the following terms:
Plan Restrictions: Depending on your specific plan, direct billing may not always be available, even for the listed insurance companies. Please check your plan details for eligibility.
No Guarantee of Services: While we strive to provide accurate billing and service availability, we do not guarantee that all services will be covered under direct billing.
Client Responsibility: In the event that direct billing does not work for any reason, you are responsible for paying the fees directly and will need to submit a claim to your insurance for reimbursement.
Authorization: You authorize the RMT to directly bill your account for services rendered, subject to the terms of your plan.
Policy Details: Please note that we cannot provide details on your total coverage, remaining balance, or specific policy limitations. We recommend checking with your insurance provider for this information before your session. Co-pay, deductibles, and/or amount owed shall be covered by the client and a physical or digital copy of the receipt will be issued by the Registered Massage Therapist.
Primary Policy: We process only one insurance policy per visit. The primary insurance coverage must be fully utilized prior to billing a secondary policy.
